SUBTLETY - Now, be careful, you might look at this (very happy) patient, and say ‘Why bother?’. Please don’t do that. That would be simplifying things too much - I’ve gone on enough about why people have cosmetic surgery, and why it is ok, so long as they are doing it just for themselves, have realistic expectations, and realise the risks, consequences etc. This is no different. So please don’t judge. This patient had a very specific request - she did not like how much her nose stuck out from her face. So, we discussed and agreed to ‘deproject’ the nose. She was also very specific that she liked pretty much everything else about her nose - it reflected her ethnicity, it was in keeping with others in her family, and she did not want to change that. It’s interesting, I’m hearing this more and more - for example, ‘I’m from Iran, but I DO NOT want a typical ‘Iranian rhino’ please! That’s too much’. This is music to my ears - means people are educating themselves, thinking, and talking their time before coming to me. This lady had a ‘deprojection’ of the tip, but if you do just that, the rest of the nose will look out of balance, so she had the rest of the nose balanced out, whilst still maintaining the bits she liked - all done using an open #piezorhinoplasty approach, combined with a ‘preservation’ technique for the rest of the nose. DO NOT HAVE PLASTIC SURGERY! Unless you have considered the following: 1. THINK. Have you have spent AGES thinking about this? Do so. Don’t rush. 2. WHY are you doing this? If you understand your reasoning, you are much more likely to be happy after. 3. WHY NOT? Think of reasons why not to go ahead. Will you really be happier after? Aren’t you happy enough now? Why should you go ahead and take all the risks, spend all that cash? 4. WHO are you doing this for? I hope it is just FOR YOURSELF. Do not have surgery for anyone else. 5. WHAT IF? What if you had problems, what if you’re disappointed? How are you going to cope? What about the extra costs? The extra, unplanned downtime? 6. WHO is doing the surgery - do your research, get to know your surgeon even BEFORE YOU MEET THEM. Also: 7. WHO WITH? Do not do this alone. Have a ’support crew’ - a relative, a close friend, or partner perhaps. Make sure they are on your side and understand. Because if things don’t go to plan, you will need them by your side. Things will be stressful so have someone around who won’t judge and make things worse. 8. WHERE? If you’re going abroad, how long should you stay there? What happens with complications? Who exactly is the surgeon? Will they listen to and understand exactly what you want? Will there be a language barrier? Is the hospital any good? ‘IF YOU’RE SAD NOW, YOU MIGHT STILL FEEL SAD THERE’ It’s all about MANAGING EXPECTATIONS when it comes to a lot of things, especially when there is a significant emotional load attached. Surgery is very much one of these things. Sometimes, I meet people who expect a whole load of things to change after surgery. That just isn’t going to happen. Yes, often patients feel great after surgery, and I hear things like ‘it’s changed my life’ but the fact is that you are still going to be the same ‘you’ but with a better nose, a younger face, nicer breasts, a flatter tummy etc. That doesn’t mean that everything else in your life is also going to be sorted. Might sound obvious, but it’s surprising how much people attach to an operation. I’m therefore always saying things like ‘what if it isn’t good enough after? How will you deal with that? You might have to live with the result forever, how’s that going to make you feel?’ I think it’s really important to have thought about these things before you commit to the operating table, and put your trust in someone like me. I know for a fact that by doing so, my patients are much happier. If I think we’re not managing your expectations properly, I’ll either refuse to operate or I’ll make you wait and think longer. HAVING REGRETS? It happens, but they do say it’s better to regret something you have done, rather than something you haven’t. This lady is not alone - she was young and impulsive, and had huge implants put in 20 years ago. She liked them at first (big was all the rage back then), but this didn’t last. 2 more kids later, some weight-gain, and they were now feeling way too heavy. She took to hiding them - wearing over-sized clothes at work, never wearing bikinis and so on (this is very similar to the stories I hear from women who have developed breasts that are too large, who then go on to have large reductions). So, we decided to down-size. But this wasn’t going to be simple - her 600cc implants still sat high and made up most of the top volume of her breasts, and she didn’t want to lose this ‘upper pole fullness’ - most women like this. Her natural breast tissue had drooped and was now too low - this created a ‘double-bubble’ effect with implant causing a bulge at the top, and the natural breast making another bulge, lower down - look at the side-view here. She needed a full-on breast reduction (removing 250g each side, with all the scars, and nipple changes), as well as smaller 310cc round implants (my usual muscle-split technique). And we got there. Here she is 3 months later. The scars have already faded well (she has been lucky here), the nipples are smaller, and her smaller implants are giving her a much better, natural shape, and proportionate size. She looked so much happier and confident when she came for her check-up. If you’re regretting a previous decision like this, and want a change, come talk to us. HERE’S A DIFFICULT ONE TO ’SHOWCASE’ Thought long and hard about posting this, but, I kept thinking that it made a good point. I am always going on about #safety and even though this shows a case of mine that had problems, it also show how we dealt with these problems. I’m really proud and pleased with the outcome. This happened 4 WEEKS after a tummy tuck, this patient suddenly started to feel unwell, she got in touch and the team was amazingly quick to escalate her care up to me. I spoke to her on the phone, and she sounded awful, so we got her into hospital that evening. She had a scan that showed fluid collected under the tummy skin, so we took her back to the operating room, opened the painful area of the incision and drained the fluid. It was probably an early infection, so by releasing it, we stopped it getting worse, collecting more and potentially threatening the overlying skin. She stayed in hospital for a few days, and one she was better (with the use of a vacuum sponge dressing), we closed the wound again. She went on to get better pretty quickly, and as you can see, she healed-up pretty well too. Her scar is still quite good, maybe not quite as good as it could have been, but I think that will settle with time. Compare that with the patient from abroad (who I posted about a while back), who lost lots of skin, got really ill, and nearly died! This is not a post to say that surgery abroad is performed badly. The problem is the distance and the communication difficulties. Unlike the previous patient, my patient got hold of me quickly, and got sorted quickly. This does not happen so often with problems from abroad, and small problems end up becoming big ones. That’s why, I advise people to STAY CLOSE TO THEOR SURGEON FOR SEVERAL WEEKS. TELL ME WHY, I don’t like Uplifts I’ve really gone off pure uplifts these last few years. No matter what’s done, the breast tissue drops back down again and all that’s left are loads of scars, and for no good reason. So, I’ve pretty much stopped doing pure ‘mastopexy’ and ‘mastopexy-augmentation’ ie an uplift with implants. The whole reason a woman needs an uplift is because: 1. The skin is stretchy 2. The breast tissue is heavy This combination results in low, heavy breast tissue and emptiness up top. The ideal solution is to find a way to lift this heavy tissue back up, remove excess skin, and keep it all there forever. Trouble is, the skin is still stretchy and the breast is still heavy. So the problem happens all over again. Lots of scars for no good reason. So, I’ve gone a little radical. I remove the heavy breast tissue and excess skin altogether by doing a breast reduction. I then replace the volume with an implant. By putting the implant under the muscle (my usual preferred technique of a muscle-split), we end up with an implant that likes to stay high-up. This is a one of the characteristics of sub-muscular implants, they sit high. This is also one of the problems with them - they can end up too high. We are taking advantage of this characteristic here. But, there’s a price to pay. There are all the downsides of a reduction (scars, nipple sensation loss, and lack of breast feeding), and the down-side of implants (need changing every 15 years ish). But at least you don’t get a ‘lift’ that lasts a couple of years at most and lots of scars for no good reason. You get a solution that should last much longer (in my opinion - other surgeons disagree and we debate this constantly!). If you think this might be for you, please do get in touch. BUYER BEWARE! There’s a lot of #bull and #hype out there. Take the ‘2cm Breast Implant Scar’. Just not possible with silicone implants. But there are those out there selling this… I just don’t buy it. I like to make small scars. Keeping scars minimal is a good idea. And I can just about get down to 3cm with small implants (this case was 210cc, muscle split) in a thin patient. But, I often have to go longer, if I am to safely insert a bigger implant. If the incision is too small the implant gets really squeezed as it’s inserted. This can damage the silicone gel permanently (gel fracture) and distort the implant such that it ends up oddly shaped forever. Not a great look!! It might even be more susceptible to rupture, so that’s not good either. A truly 2cm incision just doesn’t work. It’s too traumatic on the implant, even if a Keller Funnel is used. So it’s too risky to do. The smallest I can manage is 3cm, and that is often pushing it. I often end up increasing the length, just to be safe. So, don’t fall for the hype. A slightly longer scar is probably fine, so long as it is placed close to the breast fold, underneath the breast. Even if it is slightly longer, it will still be hidden in the shadow of the breast. REDUCTION RHINOPLASTY Rhinoplasty in most of the world tends to involve reducing the size as well as reshaping the nose. In the ‘Far East’, however, noses often need to be built up or ‘augmented’. So, like breasts, it’s also possible to have an augmentation or a reduction. 😀 Here, a lot of tissue needed to be removed. The risk here is that we are left with too much skin, which can hang down loosely. Unlike a breast, it is not possible to reduce the skin of the nose. We rely on elasticity to shrink the skin down. Where the skin might not be elastic enough, we have to compromise and reduce less. In this case, the skin was great and a really drastic reduction was achieved. This patient literally said that surgery ‘has changed my life’. You can probably see what she means. Surgery at REAL @realclinicuk is done as a ‘day case’, you go home the same day. LESS MEANS LESS With a breast reduction, we often see a dramatic change in all aspects of our patient’s life. The weight can be like carrying a backpack around on your front. Permanently. In this case the weight was 817g and 936g - almost 2 kilograms! This weight causes back ache, neck pain, sores and infections under the breasts, bra strap pain and sores on the shoulders and more. Women can’t exercise, they can’t swim, hugging their loved-ones is difficult and embarrassing. They can’t swim (because the extra fat in the breasts acts like a float). Large breasts can be a nightmare. Surgery does come with a price - nothing is perfect. There are the scars, the nipples are often numb, permanently and breast-feeding usually isn’t possible. There’s also a (very) small risk of losing the nipples altogether - but it can be reconstructed. None of this is ideal, but it is often a price women are willing to pay, and they don’t mind the risks. The relief felt after a reduction is worth it. SUPER-NATURE Here’s another natural case, using smaller, round, nano-textured implants. I used my favourite submuscular-split (SMS) technique. Results are shown at 6 weeks and things have settled really nicely. She no longer needs to wear the band, but a support bra will be her friend for life, I advise all my breast surgery patients to wear a bra 24/7 forever! Those of you who are eagle-eyed will have spotted a small dressing under the left breast. This was a tiny ‘stitch-reaction’, and a little area was still sore - about the size of a pimple, this will settle in no time. The great thing is that a small issue like this didn’t turn into something large. She emailed us with her concerns, we saw her within 24hrs and all is well. This doesn’t always happen with surgery abroad, away from home, no matter how good the surgery is or how great the hospital is. So please, stay close to home. A GOOD DAY So, we did it! After the disappointing delay last week, when we had to cancel what would have been our first case, we did 3 cases in one of our our brand new theatres today. REAL SURGERY!⠀ We did our first ‘proper’ operation yesterday under local anaesthetic at REAL. ⠀ ⠀ Thank you so much, Gary, from @the_clinic_builders for being our guinea pig! PEER REVIEW!⠀ ⠀ In medicine and science we rely on a process called ‘Peer Review’. ⠀ ⠀ This is where people who are similarly qualified to you look at your work and give an honest opinion. ⠀ ⠀ This is done for scientific publications, research, audit, and looking at complications after surgery (morbidity and mortality). It helps keep things safe, regulated and real. It stops people going too ‘off piste’. Definitely a good thing. ⠀ ⠀ With aesthetic surgery, things a are less straightforward. NOSES ARE SUBTLE, DIFFICULT THINGS⠀ ⠀ We each have our own ideas of what is ‘ideal’ or ‘perfect’ for us. Some of us are perfectly happy with the nose we were born with, others are not. ⠀ ⠀ What bugs us can be ‘obvious’ or ‘dramatic’ and having a rhinoplasty can make a mega ‘transformation’. ⠀ ⠀ Or, more often, the changes are subtle. NOT A BREAST IMPLANT CAPSULE⠀ ⠀ This is a rare complication after tummy surgery. So in theory, it could happen after a tummy tuck, I guess. So far I haven’t seen this in a cosmetic surgery patient. ⠀ ⠀ This wasn’t a tummy tuck case, it was in a reconstructive case - a patient who was on blood thinning medication (to fight blood clots). ⠀ ⠀ After abdominal surgery this patient must have had an area of bleeding. This blood was too much for the body to absorb itself (by the way, most tummy tuck patients don’t get this much bleeding, and even if they do, as I only choose to operate on fit, healthy patients, they are generally very able to deal with small blood collections). ⠀ ⠀ The blood turns into a ‘haematoma’. Haematomas then ‘organise’ by separating into liquid (seroma) and solid (cellular) parts. The liquid should then just be soaked up and the solid either gets removed by the immune system or turns to scar. ⠀ ⠀ Here there was just too much, so it stuck around for too long and the body just decided to ‘wall it off’ by encapsulating it - very similar to the soft capsule that forms around a breast implant. ⠀ ⠀ It was very big (weighed 850 grams!) and was very uncomfortable. BUBBLES AND BREASTS⠀ ⠀ One of recent posts showed how times change and you might eventually decide to have your implants removed. But it might not be simple.⠀ ⠀ This lady had implants put in quite a while ago, time moved on, she put on a little weight and her breast tissue also enlarged. ⠀ ⠀ She always felt her implants were too big, but made the most of them, so to speak - she enjoyed life (and boy has she had a life!) but now, at over 60-years-old felt it was time ‘to act her age, not her shoe size’ as a great artist once sang. ⠀ ⠀ Her implants had stayed sitting higher up, but her own breast tissue had dropped, giving a ‘double-bubble’ appearance out of a bra. In a bra she now felt huge. BETTER, NOT PERFECT⠀ ⠀ That’s all I ever offer my patients.⠀ ⠀ If I don’t think I can make things better, I won’t offer you surgery. ⠀ ⠀ I will never make things ‘perfect’ - no one is perfect and everyone’s idea of perfection is different anyway - ie it is ‘subjective’, depends on who you speak to. ⠀ ⠀ If ‘better’ isn’t ‘good enough’ for you, then you should not go ahead with surgery. It’s better to wait and think more, perhaps. ⠀ ⠀ This young lady had surgery as a child, to correct ‘pectus excavatum’ where the breast bone (sternum) goes inwards and can push on the heart. Fairly major stuff, but it left her with bumps and dips in the bone, as well as different sized breasts. ⠀ ⠀ It took a lot of working out to come up with a plan - several consults, and then lots of sizing and drawing before starting surgery. ⠀ ⠀ She had a combination of fat transfer and implants (different sizes each side). ⠀ ⠀ Things turned out pretty good I think. ⠀ ⠀ But, as always, it’s not perfect. OBJECTIVE PHOTOS (AS BEST AS I CAN MANAGE) - BREAST IMPLANTS 200cc⠀ ⠀ Here’s an example of where ‘less is more’ - a subtle breast augmentation that has increased size a little, still kept things very natural but has made a huge difference to shape and proportions, relative to the body. ⠀ ⠀ Just 200cc - these are tiny by most people’s standards! But the trend is for smaller implants these days and I think that’s a good thing. MUMMY MAKEOVERS AREN’T JUST FOR MUMMIES⠀ ⠀ (Or ‘Mommies’, if you’re separated from us Brits by a common language...)⠀ ⠀ This is a young woman who lost about 40kg (all by herself, without any Bariatric surgery) and now had a BMI of 23. ⠀ ⠀ She wanted her breasts, tummy and thighs sorting, so we undertook a full tummy tuck, liposuction to the flanks and thighs and a breast augmentation - all in one sitting, surgery took 3.5 hours. No drains, no overnight stay, home wearing a lot of compression garments. ⠀ ⠀ She’d had a difficult relationship with food since her childhood, starting off as an overweight child, then developing bulimia and losing too much weight. Plastic surgeons use ‘loupes’ to operate with - the same things that #jewellers and #watchmakers use (you see the one-eyed version in movies when they are trying to see if the diamonds from the heist are genuine!) These are mine. Had them for about a year, cost an absolute fortune (that’s Danish engineering for you), but worth every penny. They magnify everyone x3.3, so great for fiddly operating. And the light is the coolest bit - batteries last 2 hours each (I have 3) and charge using USB.Button -

Got to test out a new light to use during breast augmentations. It’s a stick on LED light - the great thing is it works with my favourite retractor (the Deaver) and it’s really bright. We usually struggle with fibre-optic lights, big cables and usually not very bright. There are disposable ones with a built in battery but even these are never bright enough. So I’ve used a head light on my loupes (operating glasses) when needed. Could be a game changer! (I have no financial interest in this device just to be clear - I should buy shares!)Button -

LIFE CHANGES⠀ ⠀ People move on. ⠀ ⠀ This lady had implants put in over 20 years ago, and now felt they were far too big. “That was a very different life back then, I’ve moved on”. ⠀ ⠀ She had been really sensible and gone for a modest enlargement for her frame. It looks like she had gone to a really good surgeon. She had also been good about wearing a supporting bra most of the time. ⠀ ⠀ As a result things hadn’t dropped very much and she managed to get a good result by just having implants removed and not need an uplift. ⠀ ⠀ She’s now really happy. “This is much more me, now”. ⠀ ⠀ We change as the years go by. Implants might be a great idea when you’re young. But later in life having a large bust might not be that important or make you feel as good as before. ⠀ ⠀ It’s also important to realise that if she had gone for crazy large implants she might have been looking at lots of uplift scars now, and maybe not such a ‘normal’ breast shape. Her tissue quality and elasticity has also stayed very good, so that has helped too. ⠀ ⠀ So it’s definitely worth planning for the future and trying to think how you might feel down the line. DO YOU HAVE REALISTIC EXPECTATIONS FROM SURGERY?⠀ ⠀ It’s really important to ‘manage’ expectations during the time before surgery and to make sure we don’t forget these after. ⠀ ⠀ This is difficult!⠀ ⠀ For example, a breast augmentation will give you bigger breasts (obviously...). But it won’t necessarily change other parts of your life. Yes, it might make you feel more confident about your body, happier in yourself and so forth. ⠀ ⠀ Surgery isn’t necessarily going to fix everything that isn’t right in your life. Again, this might seem obvious, but if things aren’t right, people look for quick fixes. And cosmetic surgery could be one of those. ⠀ ⠀ I sometimes see patients who are not necessarily just doing this for ‘themselves’ (this is the best reasons to have cosmetic surgery).⠀ ⠀ They might be trying to revive a failing relationship, maybe make themselves look more competitive in the job market, or get ‘one-up’ on their friends (I’ve heard things like ‘my mates got 300cc implants, so I need to be bigger than that’) - these are not the right reasons, I’m my opinion. ⠀ ⠀ Also, it’s important to get a realistic idea of what your actually getting, physically. See the photos here. ⠀ ⠀ With breast implants, this is relatively easy - I use these trial implants in a sports bra, and the real result mostly depends on the size of trials that are put in the bra. ⠀ ⠀ With a nose, the face, the tummy, it’s more difficult. It takes more of the ‘minds eye’ to try to work out what the result could be. Difficult. ⠀ ⠀ Even 3D simulation software can be misleading (I’ve tried it, I’m not a fan).⠀ ⠀ It’s much better to try to imagine (and expect) a ‘better version’ of what you’ve already got. RHINOPLASTY CAN CHANGE YOUR VOICE⠀ ⠀ Rhinoplasty (and especially septorhinoplasty) can alter the airway - we often perform septorhinoplasty to improve breathing. So it can have an effect on your voice, making it ‘open up’ - you rely on air escaping (and resonating) through the nose as well as the mouth when you talk/sing. ⠀ ⠀ Here’s an example - the amazing singer @jamesarthur23 - I did a septorhinoplasty on James a while back - he was a great patient, and a true gent. We even got a photo with him (that’s top anaesthetist, Dr Rich), which the kids were quite impressed by (it’s virtually impossible to impress them now that they are teens...). ⠀ ⠀ James had always been very open about wanting a rhinoplasty. And when he got surgery, he tweeted a selfie from the hospital. He also went on to say it had improved his vocal range. ⠀ ⠀ Rhinoplasty can also narrow the airway a little, so that can sometimes have an effect. It’s common to be a little blocked up and sound nasal after surgery, but this is usually temporary and you get your old voice back. ⠀ ⠀ Glad to see you’ve gone from strength to strength, James.⠀ ⠀ And thanks for agreeing to share these pics. SILICONE AND NOSES DON’T MIX⠀ ⠀ (If you ask me). ⠀ ⠀ Silicone implants are still a cheap alternative to a ‘proper’ #rhinoplasty in some parts of the world. They have gone out of fashion in the UK because of problems like this. ⠀ ⠀ This lady had had an implant a few years ago (not many) and it had started to poke out of her left nostril. ⠀ ⠀ This must have then allowed bacteria to enter the space between the skin and implant and an infection developed. There are loads of (mostly friendly) bugs in our noses, if these get on to an implant they can then multiply and cause a nasty infection. ⠀ ⠀ Here, this lady had become quite unwell. ⠀ Removing the implant and cleaning things out, along with antibiotics sorted things and she went on her way. ⠀ ⠀ Silicone isn’t a great idea in the nose, if you ask me. IT’S EASY WHEN THINGS ARE GOING FINE. ⠀ ⠀ The real test is when things don’t go so well. ⠀ ⠀ This lady had a breast augmentation in the morning. Later that evening, she was in loads of pain on the right side, and she rang up worried. The right breast was really swollen and painful. ⠀ ⠀ So we got her back in the hospital at midnight and removed a ‘haematoma’, a collection of blood and blood clot. There was an annoying, small blood vessel that was still bleeding - maybe the cauterised seal we use in surgery opened up, maybe it was in spasm at the end of surgery (and not bleeding), the spasm relaxed and off it went, slowly bleeding around the implant. ⠀ ⠀ We are always really careful to stop all bleeding, but sometimes this happens. ⠀ ⠀ Sh*t happens...⠀ ⠀ It how you deal with this that sets the quality teams apart from the cowboys. SCARS ARE FOR LIFE⠀ ⠀ This is not meant to be a flippant post - scarring affects lots of people in different ways. ⠀ ⠀ Scars are caused by lots of different things, and the visibility of a scar is a constant reminder of that ‘event’ - surgery, injury, a stupid mistake or lapse of attention, others who were less fortunate in the same event, abuse, self-harm... ⠀ ⠀ I tell my patients that there is no surgery info that does bot leave a scar. I’m now encouraging all my patients to consider a virtual consult before seeing me face-to-face, lockdown or not. ⠀ ⠀ One of the problems with face-to-face consults to start with is that they are inevitably set in a ‘hospital’. I wonder if this immediately makes patients behave more like patient and me behave more ‘doctor-like’. Perhaps sitting in your own home means that this behaviour is softened, and we are then more likely to get to know each other? I LOVE TO KISS...⠀ ⠀ I am a simple surgeon. ⠀ ⠀ I don’t like complications. The best way is to do things as simply as possible - when all these simple things come together, the result is so much better. ⠀ ⠀ We have a saying in surgery (another one...):⠀ ⠀ KISS: ‘KEEP IT SIMPLE STUPID’⠀ ⠀ If things are too complicated, they will have unpredictable consequences, interactions, and therefore, results. ⠀ ⠀ This dressing is a really good example. ⠀ ⠀ Micropore (made by 3M) is a simple, paper tape that is porous. TRIAL & ERROR - That’s what research is all about. Sometimes it works, sometimes it doesn’t. ⠀ ⠀ But we can learn stuff even when it doesn’t work - how to do things differently next time, and so on. Properly done research is never time wasted. ⠀ ⠀ I was very lucky to get a letter via my GP and the #NHS to take part in the @imperialcollege trial of a COVID Antibody Home Test Kit. ⠀ ⠀ I spent about 30 minutes doing the test, uploading a photo of the result and answering loads of questions - took ages!⠀ ⠀ But it will be worth it. ⠀ ⠀ Even if the test turns out to be inaccurate, it is important to take part in these sorts of trials where possible. The next kit tested could be the one that we all end up using. Maybe it will be this one, time will tell. ⠀ ⠀ I had COVID back in March last year, I tested positive for antibodies (IgG) a few months later and I have recently had the first dose of the Pfizer/BioNTech vaccine (again, thank you NHS). ⠀ ⠀ The result of this (research) test was positive for IgG, same as my blood test from last year. ⠀ ⠀ Who knows if this is because of my previous infection, or the vaccine. Or both?⠀ ⠀ Time will tell. JUST SAY NO!⠀ ⠀ I turn away a lot of patients. Or I ask them to take time thinking if this is really what they want doing, because I am not convinced it is right for them. And then return if they still really want surgery. And we talk some more...⠀ ⠀ So, why do I say no to some people? ⠀ ⠀ With aesthetic surgery, it basically comes down to one thing:⠀ ⠀ DO I THINK I AM GOING TO MAKE YOU HAPPY?⠀ ⠀ If the answer is no, I don’t think I should operate on you. And that’s even if you really want me to. ⠀ ⠀ If I don’t think I’ll make you happy, it means I’ll probably make little improvement or, I might make you worse. And I really don’t want to do that. And I take that to be a ‘good’ sign! ⠀ ⠀ Those who are not even slightly nervous or scared worry me. It indicates that you might not be taking things seriously. ⠀ ⠀ This is surgery, it’s not a haircut, or manicure... It’s permanent. Just scroll to the video at the end! SAFETY FIRST - YES, WE ARE OPEN, BUT...⠀ ⠀ We can’t relax too much. Yes, in London, this is ‘Lockdown 3’, here we go again etc etc, but it is more important than ever to stay safe.⠀ ⠀ There’s nothing different for us to do. Washing hands, wearing masks, social distancing, staying at home. ⠀ ⠀ BLAH BLAH BLAH, Eh?⠀ ⠀ NO!!!⠀ ⠀ This is now a race between the vaccine and the new variant of COVID that spreads more easily between people. ⠀ ⠀ Once EVERYONE is vaccinated, that’s when we can relax, not before everyone has been jabbed. ⠀ ⠀ Even if you have had the vaccine, you can still catch the virus, YOU JUST DON’T GET ILL FROM IT). ⠀ ⠀ So you can still spread it, even after you’ve been vaccinated. ⠀ ⠀ I’ve had my first dose, but I’m still wearing a mask, we are still minimising patients in the clinic. They are all sanitising their hands, wearing masks and staying 2m away from each other. IT’S NOT ALWAYS THE LOUDEST WHO NEED HELP⠀ ⠀ I’ve been reading about a thing called ‘Survivor Bias’ recently. It was a letter to a surgery journal, written by @orthopodreg that really made me think. ⠀ ⠀ Basically an out-of-touch, retired, dinosaur of a surgeon wrote a stupid letter to the journal saying ‘in my day... surgeons were trained properly, they worked long hours, never slept and were better for it blah blah....’ or words to that effect. ⠀ ⠀ This is so wrong. Whilst, yes, there are issues with modern training methods, there was also a lot wrong with the ‘good old days’. Sexism, racism were far more of a problem back then and we are thankfully improving things (slowly). ⠀ ⠀ Why do we think ‘old music is so much better than modern day rubbish’? It’s because all the crap that was released back then never gets played anymore. The strongest stuff survives. ⠀ ⠀ In a mass-casualty event, such as a bomb-explosion, you don’t immediately go to the injured who are screaming the loudest (they are well enough to be able to scream). ⠀ No, you first go check on the quiet ones, they are the ones hurt really bad (or dead). WOUND CARE, SUTURE TECHNIQUE, AND DRESSINGS MATTER!⠀ ⠀ These might all seem simple, but they are so important and should be done really well. ⠀ ⠀ I will write a book about this one day - the number of times I watch a movie and see surgery being done all wrong...⠀ ⠀ Here, we see ‘top surgeon’ (watch out for those...), Daredevil, performing minor surgery on his ex, Elektra. ⠀ ⠀ There’s so much wrong here:⠀ ⠀ I’ve never tied a knot at the end of my suture before I start stitching. I do do this when stitching buttons back on my shirts though! 😅⠀ ⠀ I make sure my knots are all ‘snug’ ie right, or else they will untie and the wound will open up...⠀ ⠀ And I always use a non-stick layer on wounds. Putting gauze straight on an open wound like this will hurt like hell when removed. ⠀ ⠀ There’s so much more wrong here. LOCKDOWN CONTINUES TO BE A CRAZY TIME⠀ ⠀ It’s been really busy these last few months. Lockdowns seem to have made everyone and their dog think about plastic surgery.⠀ ⠀ We have never been busier. ⠀ ⠀ 6 #PiezoRhinoplasty cases last week. 3 this week. A breast reduction at least once a week (We saw an awesome result today, wish the patient was happy to share photos, but alas, no, got to respect privacy), and a steady flow of #BreastImplants. ⠀ ⠀ Surgery on the #face seems to be up the most. I’m hearing that dentists are busy too. ⠀ ⠀ Staying/Working from home seems to be conducive to plastic surgery - time to hide away from the rest of the world, the masks are convenient to disguise swelling and maybe it’s that people have time as well as money burning a hole in their pocket because they’ve not got away on holiday this summer?⠀ ⠀ Whatever the reasons, we are more than happy to ‘make hay whilst the sun shines’, provided you are a good candidate and surgery is the right and safe option for you. ⠀ ⠀ As always, standards are not being compromised, and safety is paramount. THE SOLUTION TO POLLUTION IS DILUTION⠀ ⠀ Washing and showering wounds is so important. Even the most advanced, expensive dressing won’t make up for keeping a wound clean. ⠀ ⠀ (I was sort of inspired to post about this after my daughter showed me a scrape from school today, and I sent her to the bathroom to give it a good wash with soap and lots of water). ⠀ ⠀ You’re all probably fed up of hearing about #washyourhands to prevent the spread of coronavirus. Well, it’s really important. ⠀ ⠀ The same goes for your surgical wounds. I use a really simple paper tape for most suture lines. And I make you shower after just 24 hours. THE VACCINE IS A GREAT CAUSE FOR HOPE, BUT...⠀ ⠀ It’s full effects won’t kick I until next summer, earliest. ⠀ ⠀ Even if you get the vaccine, it doesn’t mean you can’t catch coronavirus. 1 IN 8 WOMEN GET BREAST CANCER⠀ ⠀ That is a lot! Think about your class at school. BAD SCARRING HAPPENS⠀ ⠀ This particular example is a ‘Keloid’ scar - it’s where the healing process goes into overdrive and the scar grows big and beyond the borders of the original wound. ⠀ ⠀ It’s one of those things. It just happens. ⠀ ⠀ We know it happens in certain parts of the body more than others and in certain skin types. ⠀ ⠀ But the bottom line is we can’t control it and we can’t really prevent it. And we don’t really understand why. ⠀ ⠀ I tell my patients that I will stitch things together as neatly as possible. A GOOD SUPPORT BRA WILL HELP KEEP BREASTS LOOKING YOUNGER⠀ ⠀ I’m absolutely convinced of this. For anyone who has a size of more than am A-Cup, I think it makes a difference. ⠀ ⠀ I’ve seen patients who had implants 20+ years ago and they followed instructions to wear a good, support bra 24/7, and still have really young looking breasts. ⠀ ⠀ It’s a bit like running with heavy shopping bags... ⠀ ⠀ If you support the bag from underneath, and not hold them by the handles, your hands and the bags survive much better. ⠀ ⠀ And you don’t end up like a young @culkamania in Home Alone. ⠀ ⠀ A good bra, to my mind, has wide shoulder straps, a good strap under the breasts, not very stretchy material, tight fitting cups , and is back-fastening (even for after surgery) - most good sports bras fit the bill. ⠀ ⠀ A good bra often doesn’t look good, but when it comes to your everyday ‘work-horse’ bras, function is way more important than looks. ⠀ ⠀ And yes, wearing nice bras (thin straps, skimpy materials etc) or even wearing no bra, is fine for the odd occasion. WHAT EXACTLY ARE YOU PAYING FOR?⠀ ⠀ I’m watching the @netflixuk documentary @thesocialdilemma and there was an amazing quote I was reminded of:⠀ ⠀ ‘In tech, we have a saying. If you’re not paying for the product, you are the product’. ⠀ ⠀ Tech companies who provide you with free access to things like social media platforms (including one that’s really close by, here...) are using us as free customers to gather data, information and knowledge about us, our habits etc. And they are making their businesses better and better. ⠀ ⠀ It’s not that different in the cosmetic surgery industry. ⠀ ⠀ You get what you pay for. ⠀ ⠀ And what are you NOT getting because you’re NOT paying for it? In other words, by going for a cheap deal, what is being left out? And, what are you being used for?⠀ ⠀ Are you being used as a guinea pig? Is your cheap and cheerful surgeon using you to learn their trade? It’s quite possible. Training up a proper #realplasticsurgeon in the UK isn’t cheap. As a result, surgery in the UK isn’t always cheap either. ⠀ ⠀ Training,in much of the rest of the world, is much shorter, and surgeons set up shop on their own very quickly, and then learn their trade! Especially when it comes to breast implants. ⠀ ⠀ But it isn’t all about size. ⠀ ⠀ WHAT ALSO REALLY MATTERS IS WIDTH! ⠀ ⠀ Because SIZE and WIDTH affect SHAPE. ⠀ ⠀ An implant that is too big will ruin the shape of a nice breast and make it look fake. I’m not at all keen on the fake look but it is something that people sometimes want. REAL PATIENTS AND THEIR REAL REVIEWS⠀ ⠀ Lockdown stopped all surgery, but we have got started again and seem to be doing ok. ⠀ ⠀ So far, we have done around 65 operations in just over 2 months. It seems like patients are behaving ‘normally’ - similar results, the usual, same occasional, little problem that need looking after (and all seem to be doing ok, with the right attention and care, as usual) and so forth. ⠀ ⠀ But, it isn’t business as usual, by any means. ⠀ ⠀ We are living in a ‘new normal’ -I’m really growing to hate that phrase...⠀ ⠀ So, there are extra precautions, such as pre-surgery swabs, social distancing in the clinic, extensive use of masks and other PPE. TRAUMA SURGERY IS FOR LIFE⠀ ⠀ With major trauma, it isn’t just a case of doing the life-saving surgery, and that’s it. ⠀ ⠀ There are often more operations to be had. ⠀ ⠀ There’s rehab - both physical (physio and occupational therapy) and psychological. ⠀ ⠀ The entire family are also affected - home might need adapting. There might be new things for everyone to learn - managing a stoma bag, doing dressings, for example. ⠀ ⠀ There will be dozens (sometimes hundreds) of visits to the hospital, to see all the various teams looking after you. ⠀ ⠀ And all of this can take ages - in this case, we are 4 years down the line from a really nasty stabbing to the belly, that almost killed this man. ⠀ ⠀ And we haven’t even touched upon the social factors here - why knife crime is on the rise in London etc. That’s probably beyond the scope of a plastic surgery account. ⠀ ⠀ And the journey hasn’t ended for him. It probably won’t really end. BREAST REDUCTION⠀ ⠀ Spoke to @glamourmag recently about this procedure. ⠀ ⠀ THIS HAS HIGHEST SATISFACTION RATE OF ALL THE AESTHETIC PROCEDURES WE DO. ⠀ ⠀ Way more than implants, more than facelifts, rhinoplasty etc etc... ⠀ ⠀ It is truly life-changing. The ability to exercise is improved, for example swimming is easier, as large breasts act as ‘floatation devices’ because of the high fat-content (Remember from school? SPOT THE DELIBERATE MISTAKE...⠀ ⠀ Here is a recent tricky case I did with liver transplant colleagues. ⠀ ⠀ This lady was left with a large hernia after a liver transplant. Often, it is too risky to repair the muscles after a transplant as it can put pressure on the new liver and risk its survival. So just the skin is closed. ⠀ ⠀ Once the transplant has settled in, we can fix the muscle, which has now developed into a hernia. ⠀ ⠀ We often use biological meshes, to reinforce the repair - these are made from processed cow or pig skin - one piece can cost as much as £15,000! They are much better and more comfortable than older plastic meshes. ⠀ ⠀ However, in this case there was a little ‘deliberate mistake’... WHY DO I WORRY?⠀ ⠀ Surgeons can be worriers and often look stressed and grumpy...⠀ ⠀ Surgery is not an exact science, so we can’t always be sure of achieving the result both surgeon and patient want. ⠀ ⠀ So, we do our best, and hope for the best. ⠀ ⠀ And then worry about it all. ⠀ ⠀ This lady had developed very different sized breasts and had an implant put in to her right side, to even them up.⠀ ⠀ Things were mostly ok to start with, but as time went by, with kids, weight changes etc, both had grown too big, and she desperately wanted the weight and size reduced. ⠀ ⠀ The old implant was now rock solid from a capsular contracture (similar to my previous post). ⠀ ⠀ This made things complicated. We can reduce breasts, and even them out, when they are different sizes. But there was also an implant here, making life even trickier. ⠀ ⠀ Things looked ok by the end of surgery but it’s often difficult to tell if all is ok when the patient is lying down. ON THE SHOULDERS OF GIANTS (AND EXTREME #PIMPLEPOPPING ...) ⠀ ⠀ This lady had breast implants put in 35 years ago, by one of my heroes. ⠀ ⠀ Brian Morgan, plastic surgeon was my very first boss when I became a doctor in 1995. ⠀ ⠀ Mr Morgan was also house officer in plastic surgery at UCH when he qualified, in 1959. I did the same job in 1995!⠀ ⠀ He was also one of the surgeons who treated victims of the King’s Cross fire in 1987 (scroll to see his painting of the fire in the last pic). ⠀ ⠀ This lady’s implants still looked pretty good 35 years later, but they had become hard and painful due to ‘capsular contracture’. So they needed changing. ⠀ ⠀ Trouble was, we had no idea what size they were, so we had dozens of different implants available to replace them. ⠀ ⠀ We used the Archimedes principle (the Greek mathematician of ‘Eureka!‘ fame), to work out the volume of the old implants. They turned out to be 200ml. ⠀ I then replaced them with slightly larger (270ml) implants to fill the resulting gap nicely. These were chosen to fit her chest and breast widths. ⠀ ⠀ The end result will take time to settle, but it should be a lot more comfortable and look much more natural. ⠀ ⠀ Hopefully these will last another 35 years!⠀ ⠀ (Huge shoutout to @zoha.ixm and @manafkhatib for working out the volumes and making these vids! I SAW A LIVING, BEATING HEART THE OTHER DAY...⠀ ⠀ Plastic Surgery isn’t just about boobs and noses...⠀ ⠀ We do a lot of RECONSTRUCTIVE procedures too. ⠀ ⠀ Here’s an example from a while back - a heart that became exposed to the world because of infection after open-heart surgery. ⠀ ⠀ It needed complex ‘soft-tissue reconstruction’. I had to use her nearby muscles (the pec majors) to fill the defect. But it wasn’t just a quick job. IT’S ALL ABOUT TEACHING & LEARNING⠀ ⠀ I think if you’re teaching you can’t help but learn at the same time. Teaching keeps you thinking about what you are doing. ⠀ ⠀ For example, today, I found myself teaching a neurosurgery trainee (I saw a living brain!) about how plastic surgeons handle tissue and how we like to obsess over suturing - fairly basic stuff but I then found myself being questioned by the trainee surgeon, and had to really think about why I do some of the simple things I’d do all the time. Made me think! ⠀ ⠀ So, I try to use every chance I get to teach (and learn)⠀ ⠀ When I appear on TV, it’s about public education - safety, taking your time before having cosmetic surgery, how to find a good surgeon etc. ⠀ ⠀ I also run seminars for fellow surgeons. I’m currently the president of the Plastic Surgery Section of the Royal Society of Medicine and we have a great webinar coming up for students, trainees and new consultants on Breast Surgery. ⠀ ⠀ In October, we have the next UK Symposium with ISAPS, the International Society of Aesthetic Plastic Surgery (I’m the UK Rep). ⠀ ⠀ I’ve taught undergrad med students for years at KCL - both through KCLPRASS, the students’ plastics society, and as a tutor for the formal syllabus. ⠀ ⠀ And I also thing Instagram is a place to teach and learn. ⠀ ⠀ It’s very easy to just show off and advertise here, but I think I also have an obligation to share the love (and knowledge!) with others. ⠀ ⠀ Please let me know if there’s anything specific you want to learn about here. IN THE MEDIA⠀ ⠀ Quite a few of the things I’ve got up to, over the years, have made their way in to the news. ⠀ ⠀ @huffpost have written about major trauma and stabbing - a lot of what we see in the NHS @kingscollegehospital ⠀ ⠀ @juliamacfarlaneabc accompanied us on what was my first charity trip to Gaza - you can see me looking really scared in her piece!⠀ ⠀ I’ve commented on new cosmetic surgery procedures, trends in surgery, strange stories, cosmetic surgery gone wrong, including tragic deaths that really should have been avoided. ⠀ ⠀ I’ve been interviewed live on the @bbcnews both to do with the Gaza work, as well as on the Victoria Derbyshire Programme (petrifying!)⠀ ⠀ My peers at the @thepmfajournal_nova recently interviewed me about my role as Plastic Surgery President @roysocmed ⠀ ⠀ And most recently (today!) I’m in @britishvogue discussing the increase I’ve seen, in demand for cosmetic surgery, in lockdown. ⠀ ⠀ Throughout all of this, I hope my message has been clear - please make sure you stay safe. THE TIMELINE OF A CRAZY BUSY DAY, POST-LOCKDOWN⠀ ⠀ I’m a little tired. ANY IDEAS? I CAN’T QUITE WORK IT OUT...⠀⠀ ⠀⠀ I’m seeing more new cosmetic surgery patients than ever, at the moment (both #VCons and #F2F)⠀ ⠀ And I’m operating on more patients than ever. Fully booked out until the Autumn - four cases today (had to open up extra operating days to meet the demand). ⠀ ⠀ I’m not expecting this to continue, once we start returning to ‘normal’ or the ‘new normal’ that everyone’s talking about.⠀ ⠀⠀ But, what I’d like to understand is: ⠀ ⠀ WHY NOW, FOR COSMETIC SURGERY?⠀ ⠀ Is it:⠀ ⠀ Money burning a hole in your pocket because you haven’t gone on your summer hols?⠀ ⠀ Time on your hands because you haven’t taken leave from work because you’ve not gone away?⠀ ⠀ Too much time working from home, more time to think about stuff?⠀ ⠀ Too much time staring at yourself on Zoom? (Requests for rhinoplasty is crazy!)⠀ ⠀ Had time to work out more, eat better, so you’re now after a bit of help from my scalpel, for stubborn areas?⠀ ⠀ Please, if you’re suddenly thinking about cosmetic surgery, or have any ideas about why, do let me know.
